Certification will enable SigmaCare to extend its leadership in medication management by allowing senior and post-acute care providers to electronically transmit prescriptions for controlled substances to institutional and retail pharmacies as well as electronically transmit prescriptions to retail pharmacies upon resident discharge. By leveraging ePrescribing, providers will be able to better serve their residents and ultimately, mitigate the risk of hospital readmissions through a streamlined medication management process and improved transitions in care.

Up until now, medication management for residents discharged from senior and post-acute care facilities has traditionally been a paper-based process, with prescribers and nurses calling in prescriptions or, in the case of controlled substances, handwriting prescriptions and providing them to residents or their caregivers to take to retail pharmacies. By connecting to the Surescripts network, as a resident is planned for discharge, medication orders can be electronically entered and approved by the prescriber within the SigmaCare EHR. Once complete, prescriptions are transmitted electronically to Surescripts which then routes them electronically to the resident’s preferred retail pharmacy for pick up.
Now, as senior and post-acute care providers along with the rest of the healthcare industry embrace ePrescribing, connecting to Surescripts will enable SigmaCare’s customers to complete the entire medication management process electronically in accordance with NCPDP SCRIPT v10.6 standard. By sending electronic prescriptions to pharmacies that are complete, accurate and validated, SigmaCare can greatly improve efficiencies for facilities, reduce medication errors and ensure that residents receive their prescriptions in a timely manner.
